oh sorry yes the form is fine it is just what get sent through to the email.. as you can see the phone number is not there.

I just realized what I updated was the framework not the template. I bought the template through Themeforest so do I go back there to download the new version.

My version is 1.0.4 and I can see the new one is 1.1.0

Yes, and the bug for the phone in the appointments form has been resolved in 1.0.6 http://updates.nicethemeswp.com/themes/paeon/changelog.txt
